Tri-County Diabetes Partnership

The Tri-County Diabetes Project is using the Friedell Committee principles in Floyd, Johnson, and Magoffin counties to strengthen the system of care and better the community one person at a time. They are led in each county by a Community Encourager who is from the county and helps keep the community moving forward.
